Senior Centers serve as the access point for community
services such as nutrition services, information and assistance, outreach
and transportation scheduling. Each County Seat town serves as the
dispatch center for the transportation service. Other services provided at
the Senior Center include nutrition and consumer education, health related
screenings as well as a wide range of social activities. The goal of the
Case Management Program for the Frail Elderly is to assist individuals to
continue living in their own home with the assistance of home and
community based services. A Case Manager will visit the individual, assess
their needs and coordinate needed services with local providers. Through
the Family Caregiver Information and Assistance Program caregivers can be directed to the
needed home and community based services to help relieve the stress of
dealing with the risks of physical and emotional burnout. Seneca
Area Agency on Aging provides advocacy services in a variety of
capacities by action taken on behalf of an older person to assist in
securing rights or benefits and can include assisting in the resolution of
complaints. Contact Seneca Area
Agency on Aging Information & Assistance and Family Caregiver
Specialist for specific
information.
